help its members make informed and enlightened decisions
regarding genetic testing.

Our commitment to our users is that genetic testing will
be free of charge and be performed by CLIA-certified
laboratories.

Our members will have full and free access to all of
flexSCAN’s services including geneVIEW and wellness360.

flexSCAN will provide each member a free and
comprehensive online personal health record for the
opportunity to store their paper medical records.

Guaranteed quality control using state-of-the-art,
validated testing procedures performed at CLIA-certified
laboratories.

Provide access to genetic testing to everyone interested

Our website and corresponding material will be
accessible, user-friendly and facilitate understanding
of information about familial implications, medical
conditions, benefits, limitations, and possible
uncertainties of test results; potential financial and
insurance considerations; and, genetic testing in
general.

We are committed to employing experts and independent
references to maximize quality and integrity.

Not a Replacement for Medical Care:
Our services are not intended to replace medical care or
the member’s physician.

Informed Consent:
Each member electing to avail themselves of the free
geneVIEW service will be adequately apprised of the
scope, benefits and risks of the services.

Promote Doctor Patient Communication:
We believe that the value of genetic testing can only be
truly realized when it is shared with the member’s
doctors and qualified genetic counselors and encourage
our members to do so.

Abnormal Results:
In the event that the genetic testing yields substantial
abnormal results, geneVIEW will withhold distribution of
the report and communicate directly with the member to
authorize distribution of the report to their primary
care provider or genetic counselor.

Quality Interpretation:
geneVIEW is committed to providing accurate,
informative test interpretation paired with explanations
in context with each member’s genetic results and health
and lifestyle concerns.

Responsible Use of Data:
We are committed to encouraging our members to be
responsible in employing their genetic testing
information into their medical and lifestyle decisions
by seeking advice and guidance from their health care
providers.

Test Results:
We will provide genetic testing results in a form that
can be provided to member’s doctors and healthcare
advisors.

Privacy:
We are committed to protecting the privacy and
confidentiality of our members through effective and
internal controls.

Network and Data Security:
We are committed to providing a secure network and
database structure by implementation of adequate
technology and controls to ensure maximum privacy of all
information obtained from genetic testing and from our
members.

Compliance:
We are committed to complying with applicable Health
Insurance Privacy and Portability Act (HIPPA) and Statue
56 of California Confidentiality of Medical Information
Act (COMA”)..

Policies:
We will provide our members access to current and
up-to-date Privacy Policy and Terms of Use.

No Third Party Distribution:
We will not, without member approval or by order of a
court with proper jurisdiction member information to
third parties.

Lab Data:
We guarantee that our lab partners destroy samples and
all identifying information following testing as
provided for by state regulations.

Compliance:
We will comply with all applicable state and federal
regulations regarding genetic testing, genetic
discrimination, and medical information.

Medical Director:
Our Physician Medical Director will ensure the accuracy
of the testing process and test interpretation.

Right to Terminate:
Members may at any time terminate their relationship
with geneVIEW and wellness360 at any time.

Your Responsibility:
You agree to provide accurate personal, medical and
family history and any medical information necessary to
maximize the best possible genetic testing outcome.

wellness360:
You understand and agree that geneVIEW’s free genetic
testing is subject to enrolling as a member “FREE OF
CHARGE” into flexSCAN’s wellness360 web site and
complete the online health risk assessment.

Possible Inclusive Results:
You understand that genetic testing results may be
inclusive and therefore may not be subject to
interpretation.
Furthermore, you understand that genetic testing
information must be considered along with many other
factors and not on their own.
